We encourage a personal visit to the nursing facility or assisted living you are considering. Here at Windsor Nursing and Rehabilitation Center of Duval, we invite you to tour our facility and meet with our Admissions Coordinator and other staff members and discuss your personal situation. Allow us the opportunity to share with you how well we can meet the needs of your loved one. We accept admissions 24 hours and day, seven days a week. Prior to admission, our admissions team will review medical records, to determine the level of care that is needed, and to develop a plan of care. This plan will be monitored and modified as necessary throughout the length of stay, whether long-term or short-term.
