The Compliance
Program has been developed to maintain and enforce a system
of accountability with Compliance Standards, Facility
Policy and Procedures, and all applicable laws and regulations
in order to prevent fraud, waste or abuse in the Medicaid
and Medicare programs.
Code of Conduct
We are committed to: (a) providing quality and clinically
appropriate care to our residents; (b) conducting ourselves
in the most professional, legal, and ethical manner
with regard to all healthcare and business practices;
and (c) complying with applicable Federal and State
laws and regulations that govern the healthcare services
that we provide.
Compliance Officer
The compliance officer for each facility is the facility
administrator.
Complaint Reporting Requirements
All employees are encouraged and have a responsibility
to assist in combating fraud, waste and abuse in Medicare
and Medicaid programs. Reporting of fraud, waste and
mismanagement shall be done without retribution and
retaliation of the individual reporting or filing a
complaint.
All employees, managers and contracted workers can
report any complaints of abuse or fraud anonymously.
Reporting Procedure:

Each
employee shall be required to follow the chain of
command when reporting any allegation/s involving
suspected fraud, waste or abuse in Medicare and
Medicaid programs: |

The Facility's
Compliance Officer shall be the first person to
contact and report any allegations of fraud, abuse
or waste. |

2. |
The second
person in the chain of command shall be the Director
of Nurses at the Facility. |
| |
3. |
The third
contact in the chain of command shall be the Hotline
Complaint Number which is posted in the Facility
and on this website. |
| B. |
Each employee has the right to REMAIN
ANONYMOUS when reporting any suspected allegations
of fraud, waste or abuse to any Medicare and Medicaid
programs. The reporting process can be either written
or verbal. |
| C. |
Employees may utilize the Complaint
Hotline to the Corporate Office to report, and file,
any allegation/s of Medicare and Medicaid fraud,
abuse or waste. The caller has the right to remain
ANONYMOUS if desired. |
| D. |
Employees may also initiate any complaint
of fraud, abuse or waste of Medicare and Medicaid
by contacting the appropriate State and Federal
Government Agencies. |

To assist and enable the investigation
of any and all allegations, certain elements or
requirements are needed for a thorough, complete
investigation, including the following: |

The Compliance Officer and all others
that receive any reports of fraud, abuse or waste
regarding Medicare or Medicaid, may keep the names
of any managers and staff members confidential who
report information when appropriate. |

Compliance Hotline
The Compliance Hotline has been established as confidential
means of reporting for employees, family members, and
residents. Callers to the Compliance Hotline can choose
to remain anonymous to report any complaints of Fraud,
Abuse, Neglect, Waste and Mismanagement. There are two
separate phone numbers for the Compliance Hotline. One
phone number is for Facility residents and family members
and one phone number is for the Employees. The Phone
Numbers to the Compliance Hotline are posted in the
facility as well as this website.
Callers who wish to remain anonymous shall be provided
with an identification number.
Proper disciplinary action, including possible dismissal,
shall be taken against anyone who retaliates against
a person who reports a legitimate concern to the Hotline.

Anti-Retaliation
Neither Your Employer nor the Facility will retaliate
against any Facility or other employee who reports suspected
misconduct in conformity with this Compliance Program.
Retaliation of an individual who has reported a complaint
or concern is not only a violation of the Compliance
Program, but it is also against applicable law.
