Description:
Candidate will screen, evaluate, re-evaluate, treat, discharge plan and make revisions in any established plan of care as supervised by the physician. This person will work with facility staff and provide support through training and in-services as needed. Candidate must provide leadership for the compassionate caring provided our residents. Description:
Develop a one-on-one relationship with each resident and family as needed for counseling. Work with family and resident on social interaction, reality orientation, and intellectual stimulation. Work with family and community resources as needed in dealing with any financial questions or problems. Counsel resident in dealing with feelings about disability, death or dying or other emotional, mental, environmental or physical limitation. Document all interactions with residents and/or family in the assessment and care plan as required by the State Standards. Is responsible for the completion of the social service portion of the Minimum Data Set within 14 days of admission
Requirements:
Must be certified or provisionally certified, by DHS as prescribed by Chapter 50 of the Human Resources Code, and have at least; (1) A bachelor's degree in social work, or a bachelor's degree in a human services field, including but not limited to, sociology, special education, rehabilitation counseling, or psychology; and (2) One year of supervised work experience in a health care setting working directly with patients.
